ABSTRACT

Objective: To investigate the clinical characteristics of patients with acute phosphine poisoning, and to follow up and evaluate the prognosis of patients. Methods: In May 2022, 12 patients with phosphine poisoning by respiratory inhalation in Beijing Chao-Yang Hospital of Capital Medical University were analyzed. The patients were treated with symptomatic support therapy. Three months later, patients were re-evaluated the symptoms of poisoning, pulmonary function and magnetic resonance imaging (MRI) of the brain to understand the prognosis of the phosphine poisoning. Results: The main symptoms of 12 patients were respiratory and central nervous system symptoms with hypoxia. The symptoms of poisoning improved after treatment. Follow-up found that the patients had different degrees of residual symptoms. Pulmonary function showed increased airway resistance. Airway challenge test was positive in some patients. MRI of the head of some patients showed small ischemic focus in bilateral frontal lobes. Conclusion: Acute phosphine poisoning may cause persistent damage to the respiratory system and central system, and residual symptoms after 3 months.

ABSTRACT

This paper reported 3 cases of poisoning caused by chlorfenagyr. Chlorfenapyr poisoning has gradually increased in clinical practice. The early stage after poisoning is digestive tract symptoms, followed by sweating, high fever, changes in consciousness, changes in myocardial enzymology, etc. Its main mechanism of intoxication is uncoupling oxidative phosphorylation. Since there is no specific antidote after poisoning, the fatality rate of chlorfenapyr poisoning remains high. The therapeutic measures are early gastrointestinal decontamination, symptomatic and supportive treatments, and early blood purification may be an effective treatment.

ABSTRACT

O Paraquat é um herbicida não seletivo altamente tóxico, sendo responsável por altas taxas de letalidade acidentais ou provocadas, devido, principalmente, à insuficiência respiratória. Apesar de a intoxicação por via oral ser a principal e mais grave, o contato prolongado com a substância em uma grande área corporal pode gerar uma toxicidade similar e levar ao óbito ­ fato pouco elucidado na literatura. Este é o relato de caso de um homem de 22 anos, que foi admitido em um hospital devido a queixas de mal-estar, náuseas, febre, cefaleia, dor abdominal, diarreia, queimaduras e dispneia. A suspeita diagnóstica inicial foi de hantavirose, leptospirose, dengue e tromboembolismo pulmonar e, posteriormente, foi comprovado intoxicação exógena com agrotóxico por via inalatória e cutânea. Exames de imagem revelaram fibrose pulmonar difusa e ele também apresentou alterações renais, hepáticas e coagulatórias. Como não há antídoto específico, foi empregado tratamento sintomático e suportivo, com uso de carvão ativado, antibióticos, corticoides, antioxidantes e hemodiálise. No entanto, o paciente teve uma piora progressiva do quadro, vindo a óbito devido à síndrome da angústia respiratória aguda e fibrose pulmonar. O Paraquat, embora proibido no Brasil em 2020, continua sendo utilizado de forma ilegal. Além disso, seu substituto, o Diquat, possui toxicidade semelhante. Assim, é fundamental que os profissionais da saúde reconheçam o diagnóstico da intoxicação por tais substâncias e suas diferentes vias de exposição. Também são necessárias novas medidas de fiscalização das substâncias e maior investimento em educação em saúde para evitar exposições acidentais, assim como relatado (AU).


Paraquat is a highly toxic non-selective herbicide and is responsible for high accidental or provoked lethality rates, mainly due to respiratory failure. Although oral poisoning is the primary and most severe, prolonged contact with the substance in a large body area can lead to similar toxicity and death ­ a fact that is little elucidated in the literature. This is the case report of a 22-year-old man admitted to a hospital due to complaints of malaise, nausea, fever, headache, abdominal pain, diarrhea, burns and dyspnea. The initial diagnostic suspicion was hantaviruses, leptospirosis, dengue and pulmonary thromboembolism, and there was subsequently proven exogenous intoxication with pesticides by inhalation and cutaneous route. Imaging tests revealed diffuse pulmonary fibrosis and he also had renal, hepatic and coagulation alterations. As there is no specific antidote, symptomatic and supportive treatment was performed using activated charcoal, antibiotics, corticosteroids, antioxidants and hemodialysis. However, the patient had a progressive worsening of the condition and died due to acute respiratory distress syndrome and pulmonary fibrosis. Paraquat, although banned in Brazil in 2020, continues to be used illegally. In addition, its substitute, the Diquat, has similar toxicity. Thus, it is essential that health professionals recognize the diagnosis of intoxication by such substances and their different exposure routes. New control measures for these substances and greater investment in health education are also needed to prevent accidental exposure, as reported (AU).

OBJECTIVES@#To study the distribution of total phosphine in phosphine poisoning victims and summarize the characteristics of phosphine poisoning cases.@*METHODS@#The phosphine and its metabolites in the biological samples of 29 victims in 16 phosphine poisoning cases were qualified and quantified by headspace gas chromatography-mass spectrometry.@*RESULTS@#Five victims among 29 were poisoned by ingestion of aluminium phosphide and 24 by inhalation of phosphine gas. Phosphine metabolites were detected in the biological samples of 23 victims, and the concentrations of total phosphine in blood ranged 0.5-34.0 μg/mL. The total concentration of phosphine in liver tissue was up to 71.0 μg/g. Phosphine was not detected in the blood of the other six survived victims, which may be related to the small amount of phosphine exposure and the delay in blood sampling.@*CONCLUSIONS@#The total concentration of phosphine in blood and tissues caused by aluminum phosphine ingestion is higher than that caused by phosphine gas inhalation. The death cases of phosphine inhalation are characterized by long exposure time, repeated exposures and age susceptibility.

ABSTRACT

Resumen Nicotiana glauca también llamada Palán Palán, es un arbusto con hojas verdes azuladas y despulidas y una flor amarilla tubular pendulante que presenta alcaloides piridínicos, como nicotina, nornicotina, anatabina y anabastina (análogo estructural de la Nicotina). Se presenta el caso clínico de una paciente de 50 años con cuadro agudo de debilidad muscular generalizada que evoluciona con paro respiratorio, tras la ingesta accidental de una cantidad desconocida de hojas de Nicotiana glauca, cultivadas en una huerta hogareña mediante técnica de hidroponía y confundidas por su conviviente con espinaca. Presentó aumento de lactato y Troponina Ultra Sensible e Hipoquinesia Global de Ventrículo Izquierdo en el ecocardiograma, compatible con Aton tamiento Miocárdico (AM), que evolucionó favorablemente. Si bien hay pocos reportes, se han informado muertes de animales y humanos, tras la ingesta accidental de Nicotiana glauca. El inicio del cuadro es rápido, con patrón bifásico, con vómitos y estímulo simpático, seguido por bloqueo ganglionar y neuromuscular, pudiendo presentar paro respiratorio, shock, convulsiones y coma. El AM es una disfunción miocárdica prolongada con retorno gradual de la actividad contráctil, posterior a un episodio breve de isquemia grave, puede ser asintomático, pudiendo presentar alteraciones en el electrocardiograma, enzimas cardíacas o ecocardiograma. Generalmente presenta pronóstico favorable, pudiendo presentar insuficiencia cardíaca ante patologías concurrentes o aumento de requerimientos de oxígeno.


Abstract Nicotiana glauca is a shrub with bluish green leaves and a pendulous tubular yellow flower. It has pyridine alkaloids, such as nicotine, nornicotine, anatabine and anabastine (structural analog of Nicotine). We present the case of a 50 years old pa- tient with acute generalized muscle weakness that evolves to respiratory arrest, after accidentally ingesting an unknown quantity of Nicotiana glauca leaves, grown in a home vegetable garden, using a hydroponic technique and confused by her cohabiting with spinach. She presented increased lactate and Ultra Sensitive Troponin and Left Ventricular Global Hypokinesia in the echo- cardiogram, compatible with Myocardial Stunned, that it evolved favorably. There are few reports, animal and human deaths have been reported following accidental ingestion of Nicotiana glauca. The onset of the symptoms is early, with a biphasic pattern, with vomiting and sympathetic stimulation, followed by ganglionic and neuromuscular blockage and may present respiratory arrest, shock, seizures and coma. Myocardial Stunned is a prolonged myocardial dysfunction with gradual return of contractile activity after a brief episode of severe ischemia, it can be asymptomatic, and it can present alterations in the electrocardiogram, cardiac enzymes or echocardiogram. Generally presents a benign prognosis, being able to present heart failure with concurrent patholo- gies or increased requirements.

ABSTRACT

In forensic toxicology, the detection of toxic chemicals from human bone marrow is often used in cases with an extended post mortem interval; however, in veterinary medicine, this practice is not used. Therefore, this study was performed to investigate the suitability of bone marrow for toxicological analysis in dogs and cats. Six animals with suspected poisoning were selected; the carcasses were sent for necropsy, and the organs were collected and preserved in buffered formalin and processed routinely for histological examination. In addition, bone marrow samples from the femur, humerus, and tibia were collected for toxicological analysis by liquid chromatography coupled to mass spectrometry detection (LC-MS). This analysis confirmed the presence of aldicarb, aldicarb sulfone, asulam, carbendazim, chlorpyrifos, dichlorvos, thifensulfuron methyl and trifloxysulfuron-sodium and associated with clinical symptoms and anatomo-histopathological alterations it was recognized the poisonings. It is expected that this study will promote the toxicological investigation of bone marrow and open avenues for the use of this tissue as an option for the detection of toxic chemicals in cases of forensic pathology.(AU)


Na toxicologia forense, a detecção de substâncias químicas tóxicas provenientes da medula óssea humana é frequentemente usada em casos com intervalo post mortem prolongado; no entanto, na medicina veterinária, essa prática não é utilizada. Portanto, este estudo foi realizado para investigar a utilização da medula óssea nas análises toxicológicas em cães e gatos. Seis animais com suspeita de intoxicação foram selecionados; as carcaças foram enviadas para necropsia e os órgãos foram coletados e preservados em formalina tamponada e processados rotineiramente para exame histológico. Amostras de medula óssea de fêmur, úmero e tíbia foram coletadas para análise toxicológica por cromatografia líquida acoplada à espectrometria de massa-massa (LC-MS). A análise por LC-MS confirmou a presença dos agrotóxicos aldicarbe, aldicarbe sulfona, asulam, carbendazim, clorpirifós, diclorvós, tifensulfuron metil e trifloxisulfuron-sódico, e em associação com sinais clínicos e achados anatomo-histopatológicos comprovou-se as intoxicações. Espera-se que este estudo promova a utilização da medula óssea como uma opção na investigação toxicológica para a detecção de produtos químicos tóxicos em casos de patologia forense.(AU)

ABSTRACT

The Amazon basin is one of the seven major geographical areas where scorpionism is recorded. In French Guiana, 90 stings per 100,000 inhabitants are registered per year. As the severity of cases is higher in children, descriptive studies are needed to have a better understanding of this pathology. The aim of the present study is to describe pediatric scorpionism in French Guiana. Methods: We conducted a monocentric descriptive retrospective study on scorpion stings in all pediatric patients admitted to Cayenne General Hospital from January 1, 2002 to December 31, 2018. Results: In this survey, 132 patients were included. Of them, 63% were male. Patients with general signs of envenomation were younger and lighter (p = 0.04). The picture was "one sting" (95.3%) by a "big" (47.6%), "black" (60%) and "small pincer" (58%) scorpion on the extremity of the body (84%). Stings occurred mainly during the day, while patients changed clothes. There was no envenomation during night. The monthly evaluation highlights that the number of stings and percentage of general signs of envenomation were closely connected to a composite variable including the variation of the level of rivers (p = 0.005). Cardiac symptoms were recorded in 82% of cases with general signs of envenomation. The presence of pulmonary; ear, nose, and throat (ENT); or gastrointestinal symptoms are related to major envenomation (p = 0.001, p = 0.01, and p = 0.02 respectively). Leukocytosis and glycemia increased according to the envenomation grade whereas serum potassium and alkaline reserve decreased. Forty-six patients needed hospitalization and seven of them required intensive care. No patient died nor presented sequelae at discharge from the hospital. Conclusion: Pediatric scorpionism in French Guiana is closely associated with child activities and climatic conditions. Severe envenomation presented most of the time with cardiac, pulmonary, and gastrointestinal symptoms.(AU)

ABSTRACT Objective: To describe the profile of children and adolescents admitted for exogenous unintentional poisoning in the emergency room and analyze factors associated with subsequent in-hospital admissions. Methods: This is a cross-sectional study based on hospital records of all subjects up to 19 years-old admitted in 2013 at a specialized toxicology service on a major public emergency hospital due to unintentional intoxication (as reported). Accidents with poisonous animals and insects were excluded. Percentages and frequencies were calculated for the qualitative variables, and measures of central tendency and dispersion for the continuous quantitative variables. Multivariate analysis was performed using binary logistic regression to identify variables associated with subsequent in-hospital admissions. Results: In 2013, 353 cases were reported. Poisonings were more frequent in children 0-4 years-old (72.5%) and in boys (55%). The vast majority was of dwellers of the Metropolitan Region of Belo Horizonte (83%), and 90% of the accidental poisonings occurred at home. 82.7% of the poisonings occurred by oral ingestion, especially of medicinal (36.5%) and cleaning products (29.4% of all poisonings). Only 12.2% of the cases resulted in hospitalization, and only one resulted in death. Residing outside Belo Horizonte (OR=5.20 [95%CI 2.37-11.44]) and poisoning by two or more products (OR=4.29 [95%CI 1.33-13.82]) were considered risk factors for hospitalization. Conclusions: Accidental poisonings occurred most frequently by ingestion of household medications and cleaning products, especially among children under 4 years-old. Preventive strategies should be primarily directed for this prevalent profile.


RESUMO Objetivo: Descrever o perfil dos atendimentos de crianças e adolescentes vítimas de intoxicações exógenas acidentais e os fatores associados às internações hospitalares. Métodos: Foi realizado um estudo transversal com base na revisão dos registros de todas as intoxicações acidentais de indivíduos com até 19 anos de idade, atendidos no setor de toxicologia de um hospital público de referência em 2013, excluídos os acidentes com animais peçonhentos e insetos. A intencionalidade da intoxicação foi baseada nos relatos. Foram calculadas percentagens e frequências para as variáveis qualitativas, e medidas de tendência central e de dispersão das variáveis quantitativas contínuas. Foi realizada análise múltipla, utilizando regressão logística binária para identificar as variáveis associadas à internação hospitalar das vítimas atendidas. Resultados: Em 2013, foram identificados 353 atendimentos em crianças e adolescentes. A faixa etária mais prevalente foi a de zero a quatro anos (72,5%), e predominaram indivíduos do sexo masculino (55%). A maioria dos atendimentos foi de pacientes residentes na região metropolitana (83%). Noventa por cento das intoxicações ocorreram nos domicílios; 82,7% se deram pela via oral, especialmente por medicamentos (36,5%) e produtos de limpeza (29,4% de todas as intoxicações). Resultaram em internações 12,2% dos casos, ocorrendo um único óbito. As variáveis associadas à internação foram: residir fora do município sede (razão de chances [OR]=5,20; intervalo de confiança de 95% [IC95%] 2,37-11,44) e o envolvimento de mais do que uma substância na intoxicação (OR=4,29; IC95% 1,33-13,82). Conclusões: O ambiente doméstico é o principal local em que ocorrem as intoxicações em crianças e adolescentes, especialmente por ingestão de medicamentos e produtos de limpeza e abaixo de quatro anos de idade. Esses achados justificam a priorização de ações preventivas direcionadas para esse perfil de acidentes.

ABSTRACT

Toxic alcohols can produce severe poisoning with multiple organic involvement and even death. The most common form is ethylene glycol. The diagnosis can be extremely difficult if there is no history of its consumption. Its clinical presentation can simulate other conditions. Ethylene glycol poisoning is characterized by an initial rise in plasma osmolal gap that decreases during the evolution, while alcohol is metabolized to acids. This last condition causes a metabolic acidosis with elevated anion gap. The clinical manifestations are diffuse neurological involvement initially, followed by hemodynamic alterations due to myocardial damage associated with hypocalcemia and acidemia. Subsequently, severe tubular renal damage appears, which may require renal replacement therapy, and finally, focal neurological alterations. To treat this poisoning, it is necessary to inhibit the transformation of alcohol into acids, increase the metabolism of the latter or withdraw them directly with hemodialysis.

ABSTRACT

Pteridium spp., popularmente conhecida como samambaia, comumente encontrada em solos ácidos, arenosos e de baixa fertilidade. A intoxicação ocorre quando os animais passam por privação alimentar ou ingestão acidental. O objetivo deste estudo é relatar a ocorrência de dois casos de intoxicação crônica por samambaia em bovinos na região nordeste do estado do Rio Grande do Sul, Brasil. Ambos os casos cursaram com sinais clínicos de disfagia, halitose, salivação e presença de tumores na orofaringe. A baixa oferta e qualidade de forragem, superlotação das áreas de pasto, manejo inadequado das pastagens, a presença de Pteridium arachnoideum, os sinais clínicos em conjunto com os achados de necropsia e o exame histopatológico confirmaram o diagnóstico de intoxicação crônica.


Pteridium spp., commonly known as fern, is commonly found in acidic, sandy and low fertility soils. Poisoning occurs when animals go through food deprivation or accidental ingestion. The objective of this study is to report the occurrence of two cases of chronic intoxication in cattle in the northeast region of the state of Rio Grande do Sul, Brazil. Both cases had clinical signs of dysphagia, halitosis, salivation and the presence of tumors in the oropharynx. Low supply and quality of forage, overcrowding of pasture areas, inadequate pasture management, presence of Pteridium arachnoideum, clinical signs, necropsy and histopathological of lesions confirmed the diagnosis of chronic intoxication.

Resumo Propor uma lista-referência de códigos da Classificação Internacional de Doenças e Problemas Relacionados à Saúde (CID-10) para a vigilância de reações adversas e intoxicações a medicamentos, denominados de eventos adversos. A elaboração da lista-referência percorreu quatro fases: definição dos códigos, validação e duas de caracterização da lista. As associações entre variáveis foram avaliadas por qui-quadrado de Pearson e análise de correspondência múltipla. Foram identificados 691 códigos relacionados com reação adversa a medicamentos (52,1%) e intoxicação medicamentosa (47,9%). Um total de 687 (99,4%) e 511 (73,9%) códigos foram validados na 1ª e 2ª validação, respectivamente. Há diferenças estatisticamente significativas (p < 0,05) entre reações adversas e intoxicação medicamentosa nas variáveis utilizadas para a caracterização da lista-referência. A associação entre medicamento e admissão hospitalar e óbito foi estatisticamente significativa quando estratificada por tipo de evento adverso (p < 0,001). Na análise de correspondência múltipla identificou-se três agrupamentos de códigos em que há associações entre as categorias de resposta das variáveis estudadas. A lista-referência pode ser uma ferramenta útil nas ações de farmacovigilância no Brasil.


Abstract ICD-10 is the 10th revision of the International Statistical Classification of Diseases and Related Health Problems (ICD), a medical classification list by the World Health Organization. It contains codes for diseases, signs and symptoms, abnormal findings, complaints, social circumstances, and external causes of injury or diseases. Associations between variables were evaluated using Pearson's chi-squared test and multiple correspondence analysis. Six hundred and ninety-one (691) codes were identified related to adverse drug reactions (52.1%) and drug poisoning (47.9%). A total of 687 (99.4%) and 511 (73.9%) codes were validated in 1st and 2nd validation, respectively. There were statistically significant differences (p <0.05) between adverse reactions and drug poisoning in the variables used to characterize the reference list. The association between drug and hospital admission and death was statistically significant when stratified by type of adverse event (p <0.001). Three groupings of codes were identified in multiple correspondence analysis where there are associations between categories of response assessed. The reference list can be a useful tool in pharmacovigilance actions in Brazil.

A infestação de pastagens por Pteridium arachnoideum é um problema que afeta a pecuária em diversas partes do mundo, ocasionando perdas produtivas e mortalidades nos rebanhos, bem como a redução das áreas de pastagens. Devido aos prejuízos na bovinocultura, foi proposto o acompanhamento de uma propriedade no Rio Grande do Sul com perdas por Pteridium arachnoideum. Realizou-se um levantamento das mortes associadas à intoxicação por samambaia na propriedade durante o período de janeiro de 2007 a janeiro de 2015. Adicionalmente, testou-se a viabilidade de ovinos como ferramenta de controle biológico de samambaia e se acompanhou as tentativas de controle da planta através do uso dos herbicidas metilsulfuron-metil e picloram, os quais eram associados ou não à roçada prévia. Das mortes por intoxicação natural acompanhadas na propriedade, observou-se 22 casos de quadro agudo de diátese hemorrágica e seis de quadro crônico de carcinomas do trato digestório superior. Em cinco bovinos jovens com diátese hemorrágica, além de hemorragias disseminadas e infartos, observou-se acentuado edema laríngeo, que cursava, clinicamente, com dispneia e estertores respiratórios característicos. Os carcinomas do trato digestório superior, apesar de menos frequentes, causaram perdas expressivas, devido à mortalidade anual de reprodutoras. Notou-se que a introdução de bovinos jovens em áreas recentemente roçadas pode resultar no consumo de grandes quantidades de samambaia e na ocorrência de surtos da enfermidade aguda. O controle das populações de samambaia pelo pastejo por ovinos não foi eficiente, devido ao baixo consumo observado com a lotação de quatro ovinos por hectare, embora mortalidade de ovinos por consumo da planta não tenha sido registrada. Após a retirada do potreiro infestado por samambaia, ao final do experimento, oito dos ovinos foram acompanhados, clinicamente, por três anos e não apresentaram nenhuma alteração. A utilização dos herbicidas resultou em uma redução da cobertura de P. arachnoideum. No entanto, essa prática necessita de estudos adicionais, já que algumas áreas não tiveram recuperação satisfatória da pastagem e o seu uso pode causar impactos ambientais e aquisição de resistência de P. arachnoideum aos herbicidas.(AU)


The infestation of pastures by populations of Pteridium arachnoideum has been a considerable and global problem to the livestock production, due to animal mortality and pasture coverage reduction. Given the impact of P. arachnoideum on cattle production, it has been proposed to monitor a beef cattle farm in Rio Grande do Sul, to assess some of the losses associated with the plant consumption and the methods employed for the plant control. A survey of cattle deaths associated with fern poisoning on the farm was carried out from January 2007 to January 2015. In addition, the viability of sheep as a biological control tool was tested. Attempts to control the plant through the use of the herbicides methylsulfuron-methyl and picloram associated or not with previous mowing were monitored. Cases of natural poisoning observed in the farm included the acute form (22), known as hemorrhagic diathesis as well as the chronic form, consisting in digestive carcinomas (6). Five cases of hemorrhagic diathesis in young cattle went along with marked laryngeal edema, which was clinically manifested as dyspnea and roaring, in addition to the classic pathological changes of widespread hemorrhages and infarcts. Even though less frequent, upper digestive tract carcinomas caused significant losses, due to annual mortality of mature cows. In occasions, when cattle were moved to newly mowed infested areas, outbreaks of acute poisoning were observed. The attempted control by sheep introduction showed to be unpractical, due to the little consumption seen at a stocking rate of four sheep per hectare. However, sheep mortality due to P. arachnoideum consumption was not recorded. The plant control method applied at the farm is based on both, the mowing of densely populated areas and the use of herbicides, namely metylsulfuron-methyl and picloram, in all paddocks. The reduction of P. arachnoideum coverage showed to be varied in different paddocks, however allowed the recovery of some grazing areas. This practice needs further studies, as some areas have not had satisfactory pasture recovery, and its indiscriminate use could lead to environmental impacts and resistance of P. arachnoideum to herbicides.(AU)

ABSTRACT

Se realizó una investigación descriptiva, observacional y prospectiva de 172 pacientes de hasta 18 años con diagnóstico de intoxicación exógena, ingresados en el Hospital Infantil Docente Sur Dr Antonio María Béguez César de Santiago de Cuba, de enero del 2015 a igual mes del 2016, a fin de caracterizarles de acuerdo con algunas variables clínicas y epidemiológicas. Como medida de resumen para las variables cualitativas se utilizó el porcentaje y los resultados se compararon mediante la prueba de la X², con 95 por ciento de confianza. Existió un predominio de los adolescentes en las edades de 14-18 años, del sexo femenino y del tipo de intoxicación voluntaria. En general, los medicamentos constituyeron el agente causal más frecuente, la mayoría de los pacientes evolucionó satisfactoriamente y el porcentaje de complicaciones fue bajo. Se recomendó continuar perfeccionando las estrategias de intervención comunitaria en los niveles primario y secundario de atención sanitaria, para lograr disminuir el número de afectados por este problema de salud.


A descriptive, observational and prospective investigation of 172 patients of up to 18 years with diagnosis of exogenous intoxication, admitted to Dr. Antonio María Béguez César Southern Teaching Children Hospital in Santiago de Cuba was carried out from January, 2015 to same month in 2016, in order to characterize them according to some clinical and epidemiological variables. The percentage was used as summary measure for the qualitative variables and the results were compared by means of the chi-square test, with 95 percent of confidence. There was a prevalence of adolescents aged 14-18, female sex and voluntary intoxication type. In general, medications constituted the most frequent causal agent, most of the patients had a favorable clinical course and the percentage of complications was low. It was recommended to continue improving the strategies of community intervention in the primary and secondary levels of health care, to be able to diminish the number of affected patients due to this health problem

RESUMO Objetivo Determinar os fatores associados aos óbitos em vítimas de envenenamento por carbamato (“chumbinho”). Método Estudo retrospectivo, epidemiológico tipo caso-controle, baseado nas fichas de notificação de intoxicação do centro de controle de intoxicações localizado na região metropolitana do Rio de Janeiro. Foram utilizadas 24 fichas de notificação de intoxicações de homens dos 20 aos 59 anos com história de envenenamento por carbamato entre 2005 e 2009. As fichas foram sorteadas aleatoriamente, respeitando-se a razão de 1:3 (um caso para três controles). A faixa etária variou de 23 a 58 anos; a média 43,83 anos. Resultados Os sintomas mais recorrentes foram: miose (OR = 1.0; IC 95%: 0,27 – 3,69. p= 1.0), sialorreia (OR = 0,83; IC 95%: 0,22 – 3,12. p= 0,78), dispneia (OR = 0,66; IC 95%: 0.14 – 3,03. p= 0,59). Conclusão Os óbitos apresentaram associação com sialorreia, miose e dispneia e uma forte associação com estertores pulmonares, broncoespasmos e os roncos pulmonares.


RESUMEN Objetivo Determinar los factores asociados a la muerte en las víctimas de envenenamiento por carbamato (“Chumbinho”). Método Estudio retrospectivo, epidemiológico de caso y control sobre el envenenamiento en los formularios de notificación un centro de control de envenenamiento localizado en la región metropolitana de Río de Janeiro. Utilizamos 24 formularios de notificación de envenenamiento en hombres de 20 a 59 años con intoxicación por el carbamato de 2005 a 2009. Los registros fueron seleccionados al azar, respetando la proporción de 1:3 (un caso y tres controles). El rango de edad fue de 23 a 58 años, promedio de 43,83 años. Resultados Los síntomas más frecuentes fueron miosis (OR = 1,0; IC del 95%: 0,27 a 3,69 p = 1.0.), Babeo (OR = 0,83, IC 95% 0,22-3,12 p. = 0,78), disnea (OR = 0,66; IC del 95%: 0:14 - 3.03 p = 0.59). Conclusión Las muertes se asociaron con babeo, miosis y disnea, y una fuerte asociación con estertores pulmonares, broncoespasmo y el ronquido pulmonar.

We used 24 notification forms of poisoning in men aged 20 to 59 years with poisoning by carbamate history from 2005 to 2009. The records were randomly selected, respecting the ratio 1:3 (a case to three controls). The age range was 23-58 years, average 43.83 years. Results The most frequent symptoms were myosis (OR=1.0; 95% CI: 0.27 to 3.69 p=1.0.), drooling (OR=0.83; 95% CI. 0.22 to 3.12 p=0.78), and dyspnea (OR=0.66; 95% CI: 0:14-3.03 p=0.59). Conclusion The deaths were associated with drooling, miosis and dyspnea and a strong association with pulmonary rales, bronchospasm and pulmonary snoring.

ABSTRACT

A participação dos metais pesados nas reações químicas do metabolismo dos peixes tornou esses elementos um dos principais focos nos estudos de contaminação. O objetivo deste estudo foi determinar as concentrações dos metais Cd, Cr, Cu, Pb, Zn e Hg no tecido muscular de peixes da bacia do rio Cassiporé (Estado do Amapá) e ambiente, visando avaliar os riscos de contaminação. As concentrações de Cd em Plagioscion squamosissimus (1,061 µg g-1) e Pb em Poptella compressa (4,280 µg g-1) apresentaram valores que ultrapassam os limites estabelecidos pela legislação brasileira. As concentrações de Cr em P. compressa (0,937 ± 0,5 µg g-1), Pimelodella cristata (0,463 ± 0,03 µg g-1) e Cyphocharax gouldingi (0,364 ± 0,12 µg g-1), bem como de Hg em P. squamosissimus (0,670 µg g-1), Pseudoplatystoma fasciatum (0,630 µg g-1), Hoplias malabaricus (0,570 µg g-1 e Serrasalmus rhombeus (0,548 µg g-1) foram acima do limite legal. A coluna d'água apresentou altas concentrações de metais pesados em todos os pontos analisados, mostrando uma contaminação dessa bacia. Portanto, essas elevadas concentrações de metais pesados no ambiente e tecido muscular dos peixes indicam um elevado grau de contaminação na bacia do rio Cassiporé e risco a saúde do homem.


Heavy metals participate in chemical reactions of fish metabolism, which makes these elements a major focus on contamination studies. The objective of this study was to determine the concentrations of heavy metals Cd, Cr, Cu, Pb, Zn and Hg in muscle tissue of fish of Cassiporé River Basin (State of Amapá) and environment, aiming at assessing the risk of contamination. The concentrations of Cd in Plagioscion squamosissimus (1.061 µg g-1) and Pb in Poptella compressa (4.280 µg g-1) had values that exceed the boundaries established by Brazilian law. The Cr concentration in P. compressa (0.937 ± 0.5 µg g-1), Pimelodella cristata (0.463 ± 0.03 µg g-1) and Cyphocharax gouldingi (0.364 ± 0.12 µg g-1) and Hg in P. squamosissimus (0.670 µg g-1), Pseudoplatystoma fasciatum (0.630 µg g-1), Hoplias malabaricus (0.570 µg g-1 e Serrasalmus rhombeus (0.548 µg g-1) were above the legal limit. The water column showed high concentrations of heavy metals in all analyzed points, showing a contamination of this river basin. Therefore, the high concentrations of heavy metals in the environment and muscle tissue of fish indicate a high degree of contamination in the Cassiporé river basin and risk to human health.

ABSTRACT

Na vigilância das populações expostas a agrotóxicos, é relevante o protagonismo da rede nacional de centros de informação e assistência toxicológica para produção de dados confiáveis sobre as intoxicações. O objetivo do estudo foi caracterizar as intoxicações por agrotóxicos reportadas a um centro de assistência toxicológica do Paraná, por meio de análise retrospectiva de fichas epidemiológicas de Ocorrência Toxicológica de 1240 indivíduos com diagnóstico médico de intoxicação aguda ou crônica por agrotóxicos, registrados no período de 2003 a 2011. Foram verificados sexo e idade das vítimas, classificação química dos agrotóxicos, e circunstância, gravidade e desfecho dos eventos. Os dados foram tabulados em planilha no Software Excel 2007 e analisados por estatística descritiva simples. A média foi de 138 intoxicações/ano. O perfil das intoxicações apontou predomínio do sexo masculino e em idade produtiva; destaque para as profissões agrícolas e a presença de trabalho infantil e do idoso; alta incidência de intoxicação por inseticidas inibidores das colinesterases e maioria dos casos notificados por unidades hospitalares; maior percentual de intoxicações e óbitos na circunstância intencional; e maior proporção de cura. No entanto, elevadas taxas de internação em terapia intensiva e a letalidade indicaram gravidade dos casos.


In surveillance of populations exposed to pesticides is important the role of the national network information centers and toxicological assistance to produce reliable data on poisonings. The aim of the study was to characterize pesticide poisoning reported to a poison control center of Parana, through retrospective analysis of Toxicological Occurrence epidemiological forms of 1240 individuals with diagnosis of acute or chronic intoxication by pesticide from 2003 to 2011. Sex and age of the victims were checked, classification of chemical pesticides, circumstance, severity and outcome of events. Data were tabulated in Excel 2007 software spreadsheet and analyzed by simple descriptive statistics. The average was 138 poisoning/year. The profile of poisonings pointed predominance of working-age male; especially the agricultural professions and the presence of child and elderly labor; high incidence of poisoning by cholinesterase inhibitors insecticides and most cases were reported by hospitals; higher percentage of poisonings deaths in the intentional condition; and high cure proportion. However, high rates of admission to intensive care and lethality indicated severity of the cases.

ABSTRACT

La intoxicación aguda por cocaína es infrecuente en lactantes y niños. Las formas más frecuentes de presentación clínica son las convulsiones tónico-clónicas en apirexia, alteraciones motoras y la excitación psicomotriz. Se presenta el caso de un lactante de 1 mes que ingresó a la Unidad de Cuidados Intensivos de Niños del Centro Hospitalario Pereira Rossell por una intoxicación aguda por cocaína. El cuadro clínico consistió en manifestaciones secundarias a un estado hiperadrenérgico generalizado, con lesión miocárdica, excitación psicomotriz, distonías cervicales, convulsiones y acidosis metabólica persistente. Presentó el test de detección de benzoilecgonina en orina positivo al momento del ingreso a la emergencia. No se pudo establecer con exactitud cuál fue la vía de ingreso de la cocaína. Es necesario un alto índice de sospecha para llegar al diagnóstico de intoxicación por cocaína en lactantes.


Acute intoxication with cocaine is infrequent in children. Generalized tonic-clonic seizures, movement disorders and agitation are the most frequently observed clinical features in cocaine intoxication. We present the case of acute cocaine intoxication in a 1 month old boy who was hospitalized in the Pediatric Critical Care Unit of Pereira Rossell Hospital Center. Clinical features were secondary to generalized adrenergic stimulation. Clinical features were myocardial damage, cervical dystonic movements, agitation and metabolic acidosis. Urine toxicology screening proved positive for benzoylecgonine upon arrival at the Emergency Unit. Cocaine´s entry into the infant was not clearly established, although it was likely to be passive exposure to coca paste smoke. A high level of suspicion is needed in order to diagnose diagnosis acute cocaine intoxication in children.

ABSTRACT

This study reports poisoning in sheep wich have received two applications of closantel in therapeutic dosage (7.5mg/kg) within a preestablished 28-day period for effectiveness formulation. A high percentage of efficiency was observed after the first and second treatment, but around 72 hours after the second administration of closantel (D30), three sheep have showed mild apathy, anorexia, diarrhea, bilateral blindness, and dilated pupils without reaction. These three animals, were observed in the following 250 days, and the bilateral blindness remained. Numerous reports of animals poisoned by closantel emphasize that overdose and/or nutritional status of the animals are key factors to occur poisoning due closantel administration. However, in this experiment the signs of poisoning (mild apathy, anorexia, diarrhea and ophthalmic disturbance), observed in the three clinically healthy sheep, occurred when they received two applications of closantel (7.5mg/kg), in a interval of 28 days, highlighting the fact of obtaining a cumulative residual effect of closantel in the animal organism, caused by two consecutive applications, may also be a predisposing factor for sheep to demonstrate irreversible signs of intoxication.


O presente estudo notifica a intoxicação de ovinos que receberam duas aplicações de closantel, na dosagem terapêutica (7,5mg/kg), com intervalo de 28 dias pré-estabelecido pela eficácia da formulação. Referente aos resultados de eficácia foi possível observar elevado percentual após o primeiro e o segundo tratamento. Cerca de 72 horas após a administração do segundo tratamento (D+30), três animais que receberam closantel demonstraram leve apatia, anorexia, diarréia e cegueira bilateral com ausência de reflexo e midríase bilateral. Estes três ovinos foram observados por cerca de 250 dias após o segundo tratamento, e a cegueira bilateral não regrediu. Os inúmeros casos de animais intoxicados por closantel descritos na literatura enfatizam que a sobredosagem e/ou qualidade nutricional dos animais, são fatores determinantes para que ocorra intoxicação destes animais pelo closantel. Entretanto, no presente trabalho, os sinais de intoxicação (leve apatia, anorexia, diarréia e a alteração oftálmica) observada nos três ovinos clinicamente sadios, ocorreram quando estes receberam duas aplicações de closantel (7,5mg/kg) com o intervalo de 28 dias, evidenciando que o possível efeito residual cumulativo de closantel no organismo animal, desencadeado por duas aplicações consecutivas, também pode ser um fator predisponente para que ovinos demonstrem sinais irreversíveis de intoxicação pelo princípio ativo em questão.
